Overview
This Learning Activity provides you an opportunity to practice implementing Performance Diagnostic Checklist- Human Services (PDC-HS). In the Module Readings you were assigned Carr et al. (2013) which includes the PDC-HS in Appendix B. You will need to identify a performance concern from your work experience and complete the PDC-HS for the specific performance concern. You should NOT use any person’s real name other than your own.

Identify and apply empirically validated and culturally responsive performance management procedures (e.g., modeling, practice, feedback, reinforcement, task clarification, manipulation of response effort).
